make a fresh Blank screen on the Powerapps Canvas App page. Go to the Insert tab, choose Gallery, and then choose the Blank vertical layout, as seen in the screenshot below.
Next, expand connectors by clicking the Connect to data link from the Blank vertical display, then choose the SharePoint Data Source. Add a fresh SharePoint connection or use an existing one.
Change the label controls like shown in the image
